New Patients
 Dr. Ballew is accepting new patients.
What to bring to your appointment:
- Insurance card
- Photo ID
- Names of your other doctors
- List of all current medications, or bring them with you.
- Recent medical records or x-rays
- If you have had orthotics or braces made in the past, please bring them in even if you do not wear or do not like them. This will help doctor find out what has worked or not worked for you.
When to Arrive: Please arrive 30 minutes before your scheduled appointment time to complete any forms. We strive to stay on schedule and understand your time is extremely valuable. New patients may require as much as 60 minutes for their initial evaluation depending on their condition.
Cancellation of an Appointment
If canceling an appointment, we respectfully request a 24 hour notice. In order to be respectful of the medical needs of other patients, please be courteous and call our office promptly if you are unable to show up for an appointment. This time will be reallocated to someone who is in need of treatment. If it is necessary to cancel your scheduled appointment, we require that you call at least 24 hours in advance. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care.
